We are excited for your family to join our family! This site is designed to make your enrollment process as easy and streamlined as possible. Please take the time to go through each step to ensure your child(ren) can start on time. All of the steps listed are necessary for us to stay in compliance with licensing and to maintain a safe environment for our students and teachers. If you have any questions along the way, please do not hesitate to contact us.
Registration Forms
Please complete all documents in the enrollment packet. Please ensure that all blanks are filled in and all forms are signed in the necessary spaces. Feel free to contact your center director with any questions you may have. Thank you in advance for your commitment to your child's education.

STEP 1
Click the Application button to complete all required enrollment paperwork.
STEP 2
Click the Handbook button to read the Future Generation Parent Handbook. Once you have read the document, please click on and sign the Parent Handbook Acknowledgement Form.

Click the Orientation button. This power-point has been created for parents who are enrolling after the new school year begins. Please view the slide show to become familiar with all aspects of our program. Once complete, please
take a moment to click through and complete a quick quiz below.

STEP 4
Please click on the Agreement button to read and sign the Future Generation Tuition Information and Agreement. Your child's weekly tuition charge will be determined by your child's agreed upon schedule. Please check with the center director regarding any questions you may have related to your child's tuition.
STEP 5
Complete the Universal Child Health Form. Go to Child Pilot Parent App and click documents. Download the document then print. Once printed submit to be completed by your child's physician. Please ensure that a copy of your child's immunization record is attached (Keep in mind that ALL children 6 months and older MUST have an Influenza vaccine before 12/31 of each school year). Forms can be uploaded on Child Pilot.
STEP 6
ONLY COMPLETE IF YOUR CHILD IS ENROLLED IN THE INFANT PROGRAM (18 months old or younger). Click the Infant Feeding Plan and be as detailed as possible, so that we can best support your child's needs.
